Carlos Rivas, PT, DPT


Carlos is a Phoenix native who received his Bachelor of Science degree in Exercise and Wellness at Arizona State University and received his Doctorate in Physical Therapy at Northern Arizona University in 2010. Carlos has experience in orthopedics, post-operative rehabilitation, geriatrics, home health, outpatient neurology, aquatic therapy, and burn patients. Not only does Carlos enjoy working with all types of injuries but he believes in providing high quality care and making sure you have a positive and enjoyable experience at his location. Carlos played soccer in college and continues to be the biggest fan of the game. At times you may see his two mascots stop by the clinic, Leo, a Goldendoodle and Messi, a German Sheppard. During his free time he enjoys barbequing, golfing, hanging out with friends and traveling around the world, especially Barcelona, Spain.

FTE-Kacie Lyding
Kacie Lyding, PT, DPT
Kacie is a native of Phoenix, AZ. She received her Bachelors degree in Kinesiology from Biola University and her Doctorate in Physical Therapy from Northern Arizona University. She has experience in a variety of settings including orthopedics, post-operative joint rehabilitation, home health, geriatrics, pediatrics, acute care, and skilled nursing. Kacie enjoys helping patients achieve their full potential. She played softball in college and has coached at both the high school and collegiate levels. She also spent two summers in Asia playing softball. In her free time she enjoys spending time with her family and friends, running, and traveling.

FTE-Melanie Mcdonald
Melanie McDonald, PT, MPT
Melanie is originally from a small town in Maine, and attended Bowdoin College to earn her undergraduate degree, graduating in 1991. Afterward, she attended the University of Delaware and was awarded her Master of Physical Therapy degree in 1996. Shortly thereafter she moved to Arizona to enjoy a little warmer weather. Melanie is married with 3 children, 2 boys and a girl, who keep her busy with school and sports. She enjoys hanging out with her friends, reading, hiking, cooking, and participating in Tae Kwon Do, in which she is a First Degree Black Belt.

Since being in Arizona, she has worked extensively in outpatient orthopedic physical therapy and in the last few years has been specializing in Balance and Vestibular disorders, as well as the LSVT-BIG program for the treatment of Parkinson’s disease. Melanie considers herself very fortunate to work with this wonderfully outgoing group of people, and feels truly blessed to be able to assist her patients with their physical therapy needs.

“Following shoulder surgery the first part of February 2013, I began Physical Therapy at Foothills Sports Medicine on Kyrene Rd., Tempe (Formally Performance Physical Therapy), the second week of March, 2013. All therapy personnel, Carlos, Kacie, Crystal and Daniel have gone above and beyond to attend to my recovery. They have treated me like family and as if they are my personal trainers.

I have nothing but high regards for their professional demeanor during this extremely difficult time for me. They all have pushed me, and pushed me to the limit. It has all paid off! My Doctor advises that I am recovering at a faster rate than expected. All of that is due to the attention that I have had and continue to receive from everyone at the facility.
