Preparing for Your Appointment

Welcome to Foothills! We’re so happy you found us. Your road to a full recovery begins now, and we’re looking forward to working with you.

We recognize that you may be in pain, recovering from recent surgery, or uncertain about what to expect in physical therapy. Our goal is to make your first appointment as simple as possible.

How to plan for your first appointment:

Step 1

Call your insurance

It’s always best to know what your responsibility will be for any services. Double check that the Foothills location you’ve chose is in-Network. If you do not have insurance, we offer affordable self-pay options. Please call your clinic location to learn more.

Step 2

Arrival Time

On average, your first visit will last about an hour. We ask patients to arrive 15 minutes early to sign-in, complete paperwork and be screened for symptoms of COVID-19.

Step 3

What to bring

Wear comfortable, loose-fitting clothing. Bring or wear tennis shoes, especially if you have a lower extremity injury. Have your photo ID, insurance card, co-payment (if applicable), and prescription from your physician (if applicable).

Please bring your new paperwork with you.

Step 4

Initial Evaluation

Your first appointment is called an initial evaluation. Your physical therapist will discuss your plan of care and answer any questions. Our main goal is help you achieve a full recovery. One of our licensed physical therapist will set goals to get you there in your treatment plan. Depending on your symptoms, the physical therapist may make a diagnosis and begin treatment almost immediately.

We want your experience to be as smooth as possible, from your first visit to your last session. Don’t be afraid to ask questions.
