Formerly Camelback Sports Therapy, we are a step above other healthcare providers, offering the latest technologies and proven therapeutic methods. Nick CD, PT, DPT
Clinic Director, PT, DPT, OCSNick earned his Doctorate of Physical Therapy (DPT) from Creighton University, and received a Bachelor’s of Science in Human Physiology from Gonzaga University. He has further training in dry needling and received his Orthopedic Clinical Specialization (OCS).
Nick enjoys treating patients of all backgrounds, but finds enjoyment returning patients to their active goals, lifestyles and independence. As a clinician, he is dedicated to growth and learning to maximize patient outcomes and better support their rehabilitation journey. Nick has special interests in orthopedics, sports, weight training, power lifting, total joint rehab, and post-surgical care.
In his spare time Nick can be found cycling, golfing, weight training, supporting Washington sports teams, and spending time with his wife and two children.

Physical TherapistDr. Derek earned his Doctor of Physical Therapy (DPT) from Northern Arizona University, where he also completed his Bachelor of Science in Exercise Science. His passion for physical therapy began during his sophomore year of high school after an injury required extensive rehabilitation. Through that experience, he spent many hours working with and shadowing physical therapists, which inspired his commitment to helping others return to the activities they love.
Derek is dedicated to helping patients move better, recover from injury, and regain the ability to do what matters most to them. He values building strong relationships with his patients and strives to create a positive, motivating environment throughout the rehabilitation process.
Originally from the Gilbert and Chandler area of Arizona, Derek enjoys staying active through sports such as softball, soccer, pickleball, and weightlifting. During his time in Flagstaff, he also developed a love for snowboarding. Outside of work, he enjoys spending time with his wonderful wife, their energetic young son, and their two dogs.

Jen came to Phoenix from Philadelphia almost 14 years ago and hasn’t shoveled snow since. She attended and graduated from Arizona State University with a Bachelor of Science in Kinesiology, and after her PT internship, she decided that this was the path she would take. Jen returned to school for her PTA license and was excited to return to the clinic and help people get back to living their best lives in this city with so much to offer.
Jen has always been passionate about helping people and has found the perfect way to blend her love for athletics and education to improve people’s function in this setting. She enjoys treating all patient populations and especially loves to teach patients how minor adjustments can make a big difference in their overall quality of life. Jen utilizes her knowledge of the human body while providing skilled hands-on treatment, cupping, IASTYM, Kinesio tape, and exercise therapy to optimize patients’ movements and help them get the desired results.

With a certification from the Titleist Performance Institute (TPI), he specializes in evidence-based approaches that enhance performance and prevent injury—particularly for golfers.
As a former college golfer, Ian brings a strong understanding of golf biomechanics to his work. He uses this experience to develop personalized rehabilitation and performance strategies for golfers of all levels, whether they are recovering from injury or looking to improve their game.
Ian is committed to creating individualized treatment plans that support each patient’s recovery, performance, and long-term health.
Outside of the clinic, Ian stays connected to the game on the golf course. He also enjoys spending time with friends and taking advantage of everything Arizona has to offer.
